A healthy mind and a healthy body—this is what we all wish for ourselves. And the New Health Primary Care Clinic at New Horizons can make that happen. The Primary Care medical staff help clients with physical exams, tests and treatments, management of chronic illnesses and diseases like diabetes, and wellness services like smoking cessation, stress management, and healthy nutrition.
The Primary Care Clinic provides services to our adult clients to promote educate and prevent basic health issues while offering diagnostic evaluation for referral. These services are rendered by nurse practitioners and physician’s assistants with physician oversight & supervision. Health conditions targeted include weight management, chronic disease management, diabetes, and smoking cessation as well as other primary care needs. This coordinated system combines medical and behavioral services to address the whole person, not just one aspect of his or her condition.
